Prior to completing the form please read the instructions carefully.

1. A brief submission must be included with this application form in order for
it to be considered. There is a section for this below.
Describe what your philosophy is on coaching Minor Hockey.

2. All applications will be reviewed. Not all applicants will receive an interview.
Unsuccessful applicants will be notified by mail

3. Successful applicants will be required to attend in house, coaching clinics

4. Successful applicants will be expected to cooperate with and recognize
      themselves as an extension of the Board of Directors.
      Successful applicants shall attend the London Junior Knights Annual
      General Meeting.

5. The intent of the program is to put the best available coaches on the ice with
      the players.
      A coach may be removed from his/her position at any time if it
      is determined they are not meeting the expectations of the Board of Directors.

6. Completed application forms will automatically be emailed to the VP of Hockey Operations.

7. To be a head coach in the London Jr. Knights Hockey you must have as a
minimum accreditation, the old NCCP Intermediate Coach certification or
NCCP Development Stream 1.

Waiver:

Applicants considered for an interview will be required to complete a police check form.

Code of Conduct:
London Rep Hockey Association Inc. is committed to providing a sport environment in which all individuals are treated with respect. During the course of all Minor Hockey activities, athletes, coaches, parents, directors, volunteers, staff, chaperones and others :

Shall conduct themselves, at all times, in a fair and responsible manner and refrain from comments or behaviours that are disrespectful, offensive, abusive, racist or sexist.
In particular, the London Rep Hockey Association Inc. will not tolerate behaviour that constitutes harassment or abuse or bullying and;
Shall avoid behaviour which brings to the Association and or its affiliates, or the sport of hockey into dispute, including but not limited to the abusive use of alcohol and/ or non medical use of drugs and;
Shall not use unlawful performance enhancing drugs or methods, nor shall they engage in activity or behaviour that endangers the safety of others and;
Shall at all times adhere to Hockey Canada, OHF and it’s Member Partner’s operational policies, procedures and any rules governing any competition in which they participate on behalf of the London Rep Hockey Association Inc. o/a London Junior Knights.
