2022-2023 RCO - Request for Carded Official

Request for Carded Official (RCO) Form - 2022-2023

This form starts the process of your approval as an LJK Team official for the CURRENT YEAR. This form must be completed EVERY YEAR for all team officials, including on ice helpers. Once submitted the RCO form will be forwarded to the LJK Board for approval before you can be involved with any team activities.

Trainers must have a valid trainers certificate. Trainers certificates are valid for 2 years.

Each LJK Team requires at MINIMUM the following 4 carded officials: Head Coach, Manager, Trainer, Assistant Coach. There is no exception to the titles or roles for these 4 positions.

You will be allowed ONE additional CARDED BENCH COACH/TRAINER and one additional CARDED NON-BENCH - VOLUNTEER ON-ICE HELPER for a total of 6 CARDED Officials. Additional CARDED OFFICALS above 4 will be charged to the team at a rate of $65.

For the 2022-2023 Season, each LJK Team can have a Maximum as follows;

Head Coach
Manager
Trainer
Assistant Coach (2)
NON BENCH - Volunteer ON ICE HELPER


All team officials including the head coach and on ice helpers must complete the following before participating in ANY team activities:

1. Yearly - Rowans Law Acknowledgement - SEE BELOW
2. Yearly - LJK Code of Conduct Acknowledgement - SEE BELOW
3. Every 3 Years - Valid VSC (Vulnerable Sector Check) Police Records Check. Police check submitted must be the ORIGINAL and not a COPY and be specific to LJK and cannot be for work or another association. 
4. Once in Lifetime - Respect in Sport Program - ACTIVITY LEADER (RIS/Speak Out Certification) - link

LJK Code of Conduct Acknowledgement 2022-23

Coaches and Team Officials are required to abide by the LJK code of conduct during the LJK 2022-23 team tryouts and Season.  Please review the following.

LJK CODE OF CONDUCT

Membership in the London Rep Hockey Inc., hereinafter called the London Junior Knights, is not a right, it is a privilege. Members must abide by the Constitution, Policies and Procedures and their behavior is expected to mirror the spirit of the Code. All members of the London Junior Knights shall respect other C.H.A., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE, and LONDON JUNIOR KNIGHTS members, officials, parents, players, team officials, volunteers and Board of Directors. Any inappropriate conduct harassment or abuse (includes profanity) directed towards game or team officials, parents, players, volunteers or Board of Directors of the C.H.A., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE, and LONDON JUNIOR KNIGHTS will not be tolerated. All members of the London Junior Knights shall respect the game of hockey and shall behave in a manner so as not to make a travesty of the game. Violation by any member of any provision of the Code of Conduct may result in an immediate and indefinite family (including player) suspension or expulsion from the London Junior Knights. In the event of an alleged violation of the Code of Conduct, the Discipline Committee shall be notified and a resolution attained as soon as practical. The Discipline Committee’s decision is final. 
Rowan’s Law Acknowledgement - 2022-2023

The Ontario Government has enacted Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 (“Act”). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation (“Sports Organization”), which includes the Ontario Hockey Federation (“OHF”), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government’s issued Concussion Awareness Resources on an annual basis. A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant registers with.

The OHF Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in the OHF.
