Hockey Development Application (PAID - On Ice Helpers)


Prior to attending any LJK sanctioned Hockey Practices or Events, all PAID Hockey Development staff must be approved by the LJK Board of Directors.

You must provide;


* Company Name and Contact Information
* Company Overview and Experience Coaching and Training Minor Hockey
* Proof of  General Liabilty Insurance for on ICE hockey development - Minimum 1 million dollars - PDF
* Police Check - Not More than 1 year old - PDF 


All applications will be reviewed. Once approved your Company name will be place under our Hockey Development Page as APPROVED for the current season.

This application must be submitted each year.

Membership in the London Rep Hockey Inc., hereinafter called the London Junior Knights, is not a right, it is a privilege. Members must abide by the Constitution, Policies and Procedures and their behavior is expected to mirror the spirit of the Code. All members of the London Junior Knights shall respect other C.H.A., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE, and LONDON JUNIOR KNIGHTS members, officials, parents, players, team officials, volunteers and Board of Directors. Any inappropriate conduct harassment or abuse (includes profanity) directed towards game or team officials, parents, players, volunteers or Board of Directors of the C.H.A., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE, and LONDON JUNIOR KNIGHTS will not be tolerated. All members of the London Junior Knights shall respect the game of hockey and shall behave in a manner so as not to make a travesty of the game. Violation by any member of any provision of the Code of Conduct may result in an immediate and indefinite family (including player) suspension or expulsion from the London Junior Knights. In the event of an alleged violation of the Code of Conduct, the Discipline Committee shall be notified and a resolution attained as soon as practical. The Discipline Committee’s decision is final. 
Rowan’s Law Acknowledgement

The Ontario Government has enacted Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 (“Act”). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation (“Sports Organization”), which includes the Ontario Hockey Federation (“OHF”), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government’s issued Concussion Awareness Resources on an annual basis. A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant registers with.

The OHF Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in the OHF.
